We do our best to make our plans easy to understand. Here’s a quick outline of some of the benefits and exclusions that apply to our in-house dental membership.

• 20% off Advanced 3D cone beam x-ray/Panoramic imaging
• 15% off most Basic Dental Procedures (Fillings, Extractions, etc.)
• 10% off most Major Dental Procedures (Crowns, Bridges, Dentures, etc.)
• 50% off clear aligner consults (A Candid clear aligner consult is normally $199 and includes diagnostic records and a proposed treatment plan with a  simulation of your final result!)
• Lab-only costs and silver diamine fluoride (SDF) are excluded from discounts
• Benefits run for one full year after you sign up
• All unused benefits expire after a year
• Initial enrollment is subject to a $50 fee
• Memberships are not refundable if any services have been rendered
• Your membership can’t be combined with any other dental insurance plan
• Discounts only apply to our office, not to procedures referred out to specialists
• Discounts are based on our Usual And Customary Rate (UCR), which may change over time
• Maximum of 6 family members per plan. All members must reside in the same household.
• 'Household members', for the purpose of additional discounted memberships, is defined as a spouse, domestic partner, child, or an individual living at the same physical address as you.
• NO yearly maximums
• NO deductibles
• NO claim forms
• NO pre-authorization requirements
• NO pre-existing conditions limitations
• NO one will be denied coverage
• NO waiting periods
